Sites with new Forms

Check list to follow when adding / updating a site with glm-member-db-leads add-on.

Setup the current fields so they can be processed and put into new leads tables.

The following fields will need to be setup using the 'Advanced > Allow field to be populated dynamically Parameter Name'

Advanced Fields: Leads
Use the group name as the 'dynamically Parameter Name'

[ ] Create the groups and interests from the forms
[ ] Make sure the forms that we want to submit data to our leads are only one page form
[ ] Replace the checkboxes in each form with the new Leads type of Gravity Form Field.
[ ] Add a new checkbox for members_ok
